Dear Preschool Families,
 
Children’s Smile Connection (CSC) is excited to offer our FREE school-based cavity prevention dental program to all students during the 2025–2026 school year. With parent/guardian informed consent, students will receive preventive dental care conveniently at school.
 
Informed Consent forms were sent home with students last Thursday. Please review, sign, and return the form to your child’s teacher as soon as possible. If you misplaced the consent form click on the link below as their website also has a printable consent form.
